You must be at least 13 years of age to register with our site.
I understand that participation in sports training, leagues, and related activities involves inherent risks, including but not limited to injury, illness, or accidents. I voluntarily assume all risks associated with participation.
I certify that the participant is physically able to engage in these activities and has no medical conditions that would prevent safe participation. I agree to inform Elite Sports Center staff of any relevant medical conditions or limitations prior to participation.
I hereby release, waive, and hold harmless Elite Sports Center, its owners, staff, coaches, and affiliates from any and all liability, claims, or demands arising from injury, damage, or loss sustained while participating in any programs or using the facility.
I grant permission for Elite Sports Center to use photos and/or videos of the participant for promotional and marketing purposes unless otherwise stated in writing.
In the event of an emergency, I authorize Elite Sports Center staff to obtain medical treatment for the participant if I cannot be reached.
